In a recent publication in BMJ Case Reports, doctors at the Shrewsbury and Telford Hospital NHS Trust have detailed three cases of vulvar necrotizing fasciitis, a rare but life-threatening bacterial infection. This infection, characterised by rapidly spreading tissue death in the vulvar region, has a high mortality rate, particularly in cases involving the vulva.

The most common cause of necrotizing fasciitis, including vulvar cases, is Group A Streptococcus (GAS) bacteria, specifically Streptococcus pyogenes. These bacteria enter the body through a break in the skin or mucosa, such as after injury, surgery, or infection.

Early signs of vulvar necrotizing fasciitis include severe pain disproportionate to the visible symptoms, redness, warmth, and swelling in the vulvar area, which spread rapidly. Fever and systemic symptoms such as diarrhea, nausea, dizziness, and fatigue may occur as the infection progresses. Later signs involve skin color changes, ulcers, blisters, black spots (necrosis), pus or oozing from the infected area.

Immediate hospitalization is required for those diagnosed with this infection. Broad-spectrum intravenous antibiotics targeting GAS and other possible bacteria are given, along with urgent surgical debridement (removal) of necrotic tissue. Supportive care for sepsis and organ function may be needed due to potential complications like shock or organ failure.

Sadly, one of the women in the reported cases died of sepsis 28 hours after being diagnosed with necrotizing fasciitis that had spread throughout her labia majora, lower abdomen, and left hip. Two of the cases were diagnosed in the emergency room, while the other two women survived their ordeal, but one required three separate surgeries and reconstructive surgery.

Reports of an increase in GAS infections, the most common cause of necrotizing fasciitis, have been noted in several countries, including the UK and US. However, comprehensive recent epidemiological data specific to vulvar cases is limited. Contributing factors generally include immunosuppression, diabetes, or delays in diagnosis and treatment.

The doctors at the Shrewsbury and Telford Hospital NHS Trust have identified 20 cases of necrotizing fasciitis treated at their hospital between 2022 and 2024, compared to 18 reported in the entire previous decade. The doctors aim to raise awareness of vulvar necrotizing fasciitis among gynecologists, who have limited familiarity with it.

In summary, vulvar necrotizing fasciitis is a rare but rapidly progressing and deadly infection, requiring urgent medical and surgical treatment. Early recognition of severe vulvar pain and systemic signs is critical to reduce mortality.
